Is this problem still open? The AP address changing certainly looks odd.. I would capture a dump of frames with 802.11 headers and check whether the beacon frames or probe responses are really coming from address ff:ff:ff:ff:ff:ff at some point.

Have you tested without using power saving in the station?
You didn't include information about the firmware version of D-Link DWL-650 card. If the station firmware is older than v0.8.2, you might try enabling a workaround for nullfunc frame ACKing. This is needed with Lucent cards to get the power saving mode working. The workaround can be enabled with 'prism2_param wlan0 ap_nullfunc_ack 1'.
